• To the  patient who is seeking Herbal formula diagnosis on line, please fill out the new patient form and sign the last pages and email it back to the office.
• Our Licensed Acupuncturist will give a TCM diagnosis and make a herbal formulas and mail the tea out the same day for you.
  The TCM internet diagnosis fee is 1st time $110. Returning $60.
• Same formula prescription order diagnosis fee is the mailing cost plus that of the herbal formula .
